| DBA (Business Registration) |

This form is to register any new business name in the County. The purpose of this form is new protect the new business name from any unauthorized users. The fiing fee is $10.00

| Application for Birth Record |
|
Copies may be issued only to the individual to whom the records pertains, one of the parents named on the record, an heir, a legal representative, or legal guardian or pursuant to a court order. Certified copies are $7.00 for the first copy and additional copies of the same record is $3.00.

| DBA Dissolution |
|
This is to dissolve any business name (DBA Registration) that is still valid. For instance if you sell your business and the new owner wants to register the business in his/her new name, this form would be needed to take the business out of the current owner's name and put in the new business owner's name. Filing fee is $10.00.
